How to make a referral to Chilmington House
Before making a referral we recommend that potential Service Users or their representatives contact us to discuss their needs in brief terms. If we feel that we can meet those needs an informal visit can be arranged to look around, meet the other people who live at Chilmington House and the staff.
If after having this visit everybody involved wishes to proceed then the Home Manager plus one other team member will start an in depth assessment of need, so that we can more accurately say whether we have the skills to support the individual and whether we think they would fit in with the rest of the group. A Care Manager will also need to give us a copy of their needs assessment.
